GENERAL DOCUMENTARY REQUIREMENTS

Loan Application Form

Must be completely filled out and signed by the Borrower.

Mayor's Permit

Must be unexpired and under the name of the Borrower.

Bank Certification & Bank Statement

Submission of at least Six (6) months Bank Statement.

Billing Statement

Billing Statement must be under the name of the Borrower.

Barangay Clearance

Must be under the name of the Business or name of the Borrower, duly signed by the Barangay Captain.

BIR Certification of Registration (2303)

Original/Updated BIR 2303 must be submitted for verification.

Audited Financial Statement for 2 years

Two (2) years AFS must be submitted duly signed by External Auditor, accredited by SEC.

Income Tax Return for two consecutive years

Two (2) years Income Tax Return received by the BIR.
